Your Initial Office Visit
Your help is most appreciated, and we ask you to follow these guidelines.
- Please plan to arrive at our office 30 minutes prior to your scheduled appointment time. (If your medical history form and your administrative form are completed before arriving, you can come 5-10 minutes before your appointment).
- In addition to the reason for your scheduled visit, if you have other concerns please let our staff know when you arrive. We will always try to accommodate these requests, but in some cases we will have to ask you to make another appointment to avoid inconveniencing other families.
- Please complete forms prior to coming for your visit.
- If you need to reschedule your appointment, please provide at least 24 hours notice whenever possible so we can offer the time to other patients. There is a $25 reschedule fee for appointments cancelled within 24 hours of the appointment date.
Health Insurance
Please bring your current health insurance information when you arrive for your appointment and notify our office staff of changes in your insurance.

We accept most insurance plans.
For those without insurance we offer significant discounts for cash payments and can work out payment plans on a case by case basis.
Online Forms – Save Time Online
If you have access to a computer before your first visit, please download and complete our patient forms and bring these completed forms with you to the office.

Health Information Portability and Accountability Act (HIPAA)
The Health Insurance Portability and Accountability Act (HIPAA) was signed into law in 1996 (Public Law 104-191). Title II, subsection “F” require processes and actions to adoption of standards for security, privacy and healthcare transactions. All employees of Start to Finish Spine Care are informed and educated about HIPAA policies.
